The Eye Clinic of Bellevue will be permanently closing
June 30, 2026.
From a desktop or laptop computer, please login to your patient portal to view and print or download your past visit notes.
Once logged in, on the left side bar click "Appointments", then "Past Appointments." Then on the right click "view visit note." Then print or download the PDF. This can be repeated for as many notes as you want. You can also download any active glasses or contact lens prescriptions by clicking on "Glasses and Contacts" at the bottom of the left side bar. Then print or download these as PDFs as well.

The Eye Clinic of Bellevue is across the street from Overlake Hospital.
From NE 8th, go straight through both the traffic lights for Kaiser (NE 10th St), and Overlake Hospital. Our driveway is the second on the right after the Overlake Hospital light.
From NE 12th, head south on 116th Ave NE. Our driveway is the third on the left.
