Office Visits
When you come in for an office visit, please bring:
Current medical insurance card
A referral from your primary care physician (if required by your insurance carrier)
Your glasses and contact lenses, if you wear them
Contact lens container or prescription indicating brand, power, diameter, and base curve (for prescribing replacement lenses)
List of current medications & dosage
Photo ID
During the visit your medical history is reviewed, your vision is checked, and your pupils may be dilated. Most people are able to drive home after dilation, but some patients have difficulty driving after dilation; if you are concerned, consider making other travel arrangements. Disposable dark glasses are available. A complete examination takes approximately an hour.

Billing and Insurance
We accept Medicare assignment and participate in most insurance programs. If your insurance requires a referral, please contact your primary care doctor and bring a copy of the referral with you when you come for your appointment.
